Community Mental Health Rehabilitation and Recovery service for Adults with Complex Needs in East Lothian

East Lothian Health and Social Care Partnership (ELHSCP) is seeking interest from potential providers for delivery of a Community Mental Health Recovery Service using the Grade 4 Wayfinder Model for graded support in accommodation (tenancies in a building provided by East Lothian Council) with associated outreach. This service will be for people in East Lothian with complex mental health needs, to prevent hospital admission or to support people upon discharge from hospital. The Service will develop or sustain person-centred support, and will include supported living and community recovery services. Providers should have experience of achieving and maintaining high quality service provision as reflected in their inspection grades from the Care Inspectorate. The key objectives for this Community Mental Health Recovery Service are:

• Graded Support: Providing an environment which “fits” the Supported Persons’ needs. Including the physical environment and staffing relative to the Supported Persons’ needs, risks identified and developing meaningful days.
• Person Centred Choice: Increasing Supported Persons’ opportunities to make choices and continue to develop coping strategies by extending support networks and developing relationships
• Meaningful Days: To facilitate Supported Persons to establish community living skills by identifying meaningful routines, interests, activities and roles.
